Hemorrhoid Treatment: How to get rid of Hemorrhoids

Piles (or Hemorrhoids) are one of the best-described diseases in medical history. The oldest, credible descriptions of haemorrhoids date back to 1500 BCE in Mesopotamia. But there are even older records in Indian and Chinese literature, while we do not know accurately how old these are.

Hemorrhoids are enlarged, congested veins just under the surface tissue of your anus (the opening of your bowel) or your rectum (the last part of your bowel). They are often described as varicose veins formed in the lower rectum or around the outside of the anus. The term when used in a clinical sense, refers to the downward displacement or internal disruption of the vascular-rich connective tissue cushions located within the anal canal.

The rectum is the last part of the bowel that leads to the anal canal (the opening at the end of the bowel) where fecal matter leaves the body. Everyone has haemorrhoidal tissue within the anal canal and perianal area (area of the body surrounding the anus) that’s made up of blood vessels, connective tissue, and some muscle. These “anal cushions” act as a stop in the back passage, just like the tap washers. They don’t always become enlarged or distended or otherwise symptomatic, but we use the term for symptomatic or pathologic haemorrhoids. Internal haemorrhoids are located in the upper part of the anal canal (above a mucosal fold called the dentate line or pectinate line), and external, located below.

Internal hemorrhoids develop within the rectum and are characterised by bleeding and varying degrees of prolapse, and may be followed by pain, itching, mucus discharge, and faecal seepage. External haemorrhoids usually only give major symptoms when they thrombose(blood clots inside them ) and are then very painful.

Hemorrhoids (Piles) - Treatments

The treatment of piles depends on the type of symptoms and severity affecting the patient. The conservative treatments include Oral medications or topical suppositories.

More definitive measures or surgical techniques are needed for severe Hemorrhoids, such as when clots frequently form in external hemorrhoids, or Iigation fails to treat internal haemorrhoids or when there is persistent bleeding or when a protruding haemorrhoid cannot be reduced.

Surgical techniques under general anaesthesia include:

Stapled hemorrhoidopexy: In an effort to evade some of the postoperative pain associated with hemorrhoidectomy, stapled hemorrhoidopexy (also known as stapled hemorrohoidectomy) which is a more recently developed option that uses a stapling device to remove hemorrhoidal tissue. Surgeons recommend this procedure for patients whose hemorrhoids have prolapsed or become abnormally large, or for patients who have shown little or no improvement with nonsurgical treatment.

This procedure involves circular stapling device which takes away some of the tissue located upstream from the hemorrhoids, thus pulling the hemorrhoids upward and returns that to its normal position, and staples to the wall of the rectum. This will stop the blood flow to your hemorrhoid and decrease swelling so that the tissue will shrink and be reabsorbed.

Benefits:

  • Stapled hemorrhoidopexy requires no external incision hence considerably reduces the chances of infection.
  • Patients typically experience less pain than those who undergo traditional hemorrhoid surgery and can resume work earlier.
  • They may also experience less bleeding, swelling, and itching around the anus and inside the rectum.
  • In addition, stapled hemorrhoidopexy may reduce the chance of incontinence after the procedure.
